Position Summary:
Privia Health Advisors are home-based RN nurses, pharmacists, and other top health professionals who serve as trusted Advisors to Privia Members, providing phone based care management, wellness coaching, and health concierge services. Personal Health Advisors help to coordinate care between our Members and their primary care physicians and specialists, ensuring all parties have the right information they need to make the best clinical decisions. Personal Health Advisors support Privia Members through developing an annual Health &Wellness Plan, providing targeted medical education, answering medical questions, reviewing medical records, developing a care plan to address preventative, acute, or chronic health issues, coordinating information between PCPs and specialists, and generally helping our Members navigate the healthcare system so that they may receive the very best care and wellness support possible.
Personal Health Advisors are part of a larger team of Privia health professionals who support our Members and physician partners. Privia Health professionals include RNs, pharmacists, nutritionists, fitness experts, case managers, social workers, health plan and billing experts, MDs, and others all there with one objective only, to help our Members achieve their health, wellness, and lifestyle goals.
The relationship between a Member and their Personal Health Advisor is a special one our Members place a great deal of faith and trust in our Advisors to help them with one of their most precious assets, their health. As such, Privia is looking for highly experienced health professionals who are committed to uncompromised excellence, exemplary customer service, and tangibly improving the lives of others. Key Responsibilities:
Conduct regular assessments with Members to identify individual health and wellness needs.
Develop an Annual Health &Wellness Plan to address Member s specific goals and objectives.
Develop and implement care management plans.
Consult and coordinate with internal and external health team members to assess, plan, implement, and evaluate patient care plans.
Provide targeted health education to Members and thoroughly research and answer their medical questions.
Assist Members with finding appropriate providers and care solutions and coordinate priority appointments.
Handle inbound calls, emails, faxes, or chats from assigned Members who are seeking information and conduct outbound communications for follow-up and care coordination.
Record Member data in Privia s web based CRM and medical record system.
Research information online and in Privia s internal knowledge databases.

Requirements
Qualifications:
Graduate of accredited school of nursing with a current, unrestricted RN license in your state of residence.
Minimum 3-5 years of recent clinical experience.
Customer service oriented.
Clear, confident communication and listening skills.
Polite and friendly with an excellent phone demeanor.
Self-motivated and self-disciplined.
Demonstrated problem-solving and decision-making skills, highly resourceful.
Detail-oriented, ability to multitask, strong sense of urgency, and a commitment to excellence.
Computer competency within a Windows environment with strong typing skills.
Accessibility to a home office environment, as outlined below.
Desired Qualifications:
Disease management, case management, health plan, home health, or wellness coaching background.
Health advocacy experience.
A passion for delivering exceptional customer service and a spirit of generosity.
A strong entrepreneurial spirit.
Works well as a team member in a virtual environment.
Commitment to learning and complying with privacy standards.
Clinical experience and expertise in one of the following specialty areas is a plus: diabetes, geriatrics, cardiology, cancer, rheumatology, respiratory, allergies, pediatrics, or mental health.
Management and training experience with strong leadership competencies.
Customer contact center experience is a plus.
Bilingual skills are highly desired.
Home Office Requirements:
With the many special advantages that come with the opportunity to work from home, we do have some specific requirements regarding your home office:
A separate room to be used exclusively as your Privia office - the office should have locks on the door to ensure data security and must provide a silent environment conducive for receiving business calls, free from any distractions while you are working.
Telephone Access you must be able to provide a phone line dedicated to Privia, separate from your regular home phone line. The dedicated line must be a wired landline with no additional features. Privia will reimburse you for the monthly expense of the phone line.
High Speed Internet Access - you must have access to a dedicated high speed Internet line with speeds of 1.5 Mbps or more. DSL will work, but cable modem, or Fiber Optic (FIOS) lines are better. Dial up, Satellite, and Wireless Connections are not allowed. Privia will reimburse you for the monthly expense of the Internet line.
A laptop computer, headset, printer, and shredder will be provided to you for exclusive use with your Privia work, and remain the property of Privia Health.
