RESPONSIBILITIES/REQUIRED SKILLS/EXPERIENCE The Continuum of Care Manager is a part of the Program Design and Sales Support team in Medical Operations. The Continuum of Care Manager will support the PM regions, Dedicated Units and Specialty Program areas to ensure the quality and consistency of the case management program across all areas. Specific Responsibilities include but are not limited to: - Monitor and assess CM program performance and collaborate with regional PM areas to achieve targeted results. - Liaison between Medical Operations, Health Care Delivery and Specialty Programs. - Facilitate communications between Medical Operations, PM and Specialty areas. - Single point of contact for Medical Operations on CM programs. - Maintain training content to support program design. - Represent Medical Operations and program areas with new customer/program implementations. - SME for systems projects and reporting related to support of the CM program The selected candidate will have the following: 1. Bachelors Degree in Nursing or healthcare related field. 2. Five years experience in medical management. Management experience preferred, case management experience a must. 3. Cross functional knowledge and experience with multiple Aetna systems, programs and services. 4. Ability to manage multiple projects in a deadline driven environment. 5. Solid understanding of Aetna Medical Management programs and program design. 6. Demonstrated capability to collaborate with multiple business partners across the organization.
Position may be based in any Aetna office. Work at Home will be considered.
