As we are expanding, we are looking for an articulate professional to oversee Medicare/Medicaid community cases while delivering Evercare products to the Wisconsin market. Your case management experience as well as your Medicare and Medicaid knowledge would be valued in this position. Coordinating care plans for each manager
Updating the Nurse Practitioners (NPs) and physicians
Implementing care plans by coordinating needed services
Communicating issues to senior level management
Qualifications .
The candidate must be an RN with a current, unrestricted license in the state of Wisconsin. The RN should also have a solid history in a clinical setting (acute, home health, managed care, med-surg, long-term care or hospital). A case management history is imperative. Strong time management skills are essential as is the ability to manage case loads and travel effectively. Lotus Notes and solid typing skills are necessary. Proficiency in MS Office applications is needed. The ideal candidate will be member-focused and have the ability to collaborate with other staff members. Community-based experience is preferred. Medicare/Medicaid knowledge is ideal. Certified Case Managers (CCMs) are encouraged to apply. Bilingual Spanish skills are a plus.
