In this role you would work from home and in the field, meeting with staff in acute hospital facilities to coordinate and expedite care. Primary responsibilities include:
Conducting onsite clinical chart assessments
Identifying and evaluating delays in care
Facilitating discharge planning
Arranging alternative care services
Discussing cases with treating physicians and other healthcare professionals
Determining need for intervention, and discussing with Case Managers and physicians
Participating in rounds with the Medical Director
Cadidates must possess:
A current and valid RN, PA or APRN license in the state of New Jersey
Recent clinical experience in an inpatient/acute setting
Solid computer skills
Ability to build and maintain relationships
Valuable assets include:
Discharge planning and/or chart review experience
Background with UR in a managed care environment
M&R familiarity
