Our new Care Manager will communicate with enrollees on a regular basis to make sure they understand their benefits and treatments, so that their care plans run smoothly and effectively. This is a great chance to enjoy regular hours as you directly impact the well being of our customers.
Primary responsibilities include:
Spending the majority of each day in telephonic communication with enrollees who have recently been discharged from the hospital
Educating enrollees about their plans of care, benefits and medications
Providing information and case management to patients in an effort to prevent hospital readmission
Informing the Medical Director about any gaps or issues in the care plan
Presenting cases to the Medical Director at a weekly team meeting
Attending staff meetings as needed
Qualifications .
A current, unrestricted RN license in the state of Illinois is required, as is previous clinical experience. Case Manager certification is desirable. A background in care coordination, discharge planning and/or utilization review would also be a huge plus.
