Evercare offers a variety of services to help people in all phases of long-term care remain self-sufficient and independent for as long as possible. The company is part of Ovations, a family of UnitedHealth Group businesses dedicated to improving the health and well-being of Americans ages 50 and older.
This is a virtual career (work from home) and can be based anywhere in the DFW Metroplex.
Positions in this function require education/certification/licensure as a Social worker . Positions is responsible for providing social work oversight and problem solving, including case management and assistance with determining discharge alternatives. May identify, coordinate or provide options regarding appropriate levels of care under the direct supervision of an RN or MD. Generally work is self-directed and not prescribed.- Works with less structured, more complex issues.- Serves as a resource to others.
Qualifications .
Licensed Social Worker in the State of Texas
Previous experience working with Medicaid population
Ability to communicate internally and externally with all levels of employees including Senior Management
Strong written and verbal communication skills. Knowledge and ability to document processes and practices.
ability to work independently prioritize assignments and meet deadlines
Multitask
Experience in Windows, Microsoft Word, Excel etc
Bilingual in Spanish and/or Vietnamese preferred
