EDUCATION
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS
- Registered Nurse (RN) is required

FUNCTIONAL WORK EXPERIENCE
- Nurses: Disease management

POSITION SUMMARY
- The Disease Management Nurse applies the standard elements of assessment, planning, implementation, coordination, monitoring and evaluation and will use state-of-art information tools.
- Bilingual in Spanish a plus.
- Willingness for flexible hours desired.
- This position is for an afternoon/evening shift.
- Work from home may be available to qualified employees after 1 year of service.

Required Skills:
REQUIRED SKILLS
- Adapt member communications to support life stage and multicultural benefit needs
- Apply intervention assessment tools
- Conduct individualized clinical assessments of members care needs
- Develop patient chronic disease care management plans
- Interpersonal Communication
- Listening

DESIRED SKILLS
- Registered Nurse with current state license and a minimum of three years recent clinical nursing experience.
- Ability to exercise independent and sound judgment in decision making, utilizing all relevant information to effectively influence behavior.
- Strong verbal and written communication skills.
- Organizational skills, effective time management, ability to manage multiple priorities, and strong teamwork skills.
- Candidate must possess strong computer and keyboarding skills.
- Knowledge of managed care, eTUMS, Healthwise Knowledgebase and Disease Management preferred.
- Microsoft Outlook
- Provide members with health care information to drive member understanding of his-her health condition(s) and possible options
- Refer members to alternative healthcare programs (i.e. disease management, case management, vocational rehab, etc.)
- Review and evaluate clinical history to identify needed medical services
- Leveraging Technology
- Ensuring Ethical Decisions
