As a Nurse-Case Manager you will be responsible for assessing, planning, implementing, coordinating, and monitoring health care services to meet an enrolled members healthcare needs through communication and available resources to promote quality, cost-effective outcomes. This position works collaboratively with providers, social agencies, community resources, and a patients personal support network to assist patients and their families in accessing medically appropriate services, optimizing effective self-care strategies, identifying evidenced based treatment options and making informed decisions that reflect their healthcare needs and personal priorities.
Essential process knowledge required to perform duties includes:
-ASSESSMENT: Collecting in-depth information regarding a patients clinical history, prognosis, treatment plan, response to treatment, access to care, access to personal and community resources, utilization of care, personal coping mechanisms, learning needs and financial constraints. This information provides the foundation for the identification of patient problems and the development of a comprehensive care plan that will address identified needs.
-PLANNING: Determine specific short and long-term goals, objectives, and interventions with accompanying timeframes based upon evidence-based medicine guidelines.
-IMPLEMENTATION: Identify and carry out the relevant interventions that will facilitate the accomplishment of goals established in the care plan.
-COORDINATION: Identify, organize, and integrate the appropriate acute, rehabilitative, sub-acute, homecare, alternative, and community resources needed to accomplish the identified goals in the care plan.
-MONITORING: Collect sufficient data from all relevant resources to determine the effectiveness of the care plan.
-EVALUATION: Make repeated assessments of the care plans effectiveness at appropriate intervals and make plan modifications, as necessary, to achieve patient goals and improve outcomes.
Requirements:
3 to 5 years surgical/medical nursing experience.
Experience
Previous experience providing case management support
While this position will provide generalist
case management support to our patient/members, having experience in one of the following specialty areas is preferred:
Oncology/Neuro/OB-GYN-Pediatrics/Cardiac-Circulatory/Respiratory/Endocrine/Gerontology
Telephonic counseling experience preferred.
Demonstrated clinical performance that meets or exceeds the performance standards for the product managing.
Demonstrated problem solving skills; creatively works toward solutions.
Excellent communication and interpersonal skills.

-A training and performance evaluation period must be succesfully completed to take advantage of our telecommuting program.
Certification Requirements
Active KY RN License
- CCM Certification Preferred or eligible to obtain
Education Requirements
ASN or BSN preferred
