Disease Manager/Registered Nurse
, Due to our recent growth, Case Management Specialists, Inc has an opportunity for a high-energy, self motivated RN looking for a change in their daily routine. Case Management Specialists, Inc. (CMS) is seeking a registered nurse with experience primarily in Disease Management to join our growing medical management team. This care manager will be responsible for coordinating our Disease Management program while assisting with other medical management services. This is a telephonic position which requires excellent communication skills both written & verbal as well as computer skills to include Microsoft Outlook, Excel & Word.
The care manager position requires a registered nurse skilled in patient education and coordination of services to enhance the patients’ quality of life and to promote better control of their disease process. This position offers a rewarding and interesting specialty in nursing, which allows the nurse to advocate for the patient while providing education about their health care options and the management of their chronic illness.
Preferred candidates will have the following:
Disease management experience, primarily in diabetes, asthma, hypertension, CHF, COPD and CAD.
A firm clinical foundation, 5+ years (to include 2 years in the acute care setting) and additional experience in home care, cardiac or pulmonary rehab and/or patient education. Other related clinical experience will be considered.
Strong computer and written/oral communication skills

Our Hartland office is located in western Waukesha County. Training would occur in this office. There would be an opportunity to work from home once training period is complete. The training period would range from 4 months to 1 year, depending upon your experience and learning curve. Please note that our company does not offer health insurance.
