In addition, you would enjoy a Monday through Friday work schedule during normal business hours and no weekends! You would be overall responsible for case management with members from evaluation to post transplant care.
Please note: Since this is a work-from-home position, the individual in this role can work from anywhere in the country.
Primary responsibilities include:
? Coordinating care with providers for discharge planning and ensuring a smooth transition of care
? Educating members on resources available as well as their medical care
? Communicating an outpatient care plan to medical staff and care givers as necessary
? Handling an assigned case load
This role requires a Registered Nurse with a current license from any state. A Bachelor of Science degree in Nursing is essential. Transplant experience is also required. MS Office proficiency is a must. Managed care experience is preferred, as is proven experience working independently.
