Job Description: :
Medical Case Management is a proactive process, in which highly-skilled registered nurses collaborate closely with the patient/family, health care providers, and payer to identify and achieve functional and timely outcomes. This is done by aligning health care resources as well as maximizing efficiency, effectiveness, and communication.
Mileage paid to &from assignments. Base plus bonus - Work from home or in the office.
