DESCRIPTION .
Our Care Coordination Dept. in Medical Affairs is currently seeking Complex Case Managers. The position is Monday through Friday, 10am to 6:30pm with the opportunity to work from home after six months if job performance is satisfactory. Position responsibilities include identifying members who can benefit from KMHP s services and monitor their progress. Support and carries out the Mercy Mission. .
REQUIREMENTS .
Qualified candidates must be a licensed PA RN, 3+ years Case Management/clinical experience. (Eligible to obtain a CCM within one year of employment). Experience in Pediatrics, Home Care, assessments and Care Plan development a plus. Good computer, communication and organizational skills required. .
