DESCRIPTION .
The Special Care Unit Case Manager is responsible for identifying members who will benefit from care coordination services, perform a comprehensive assessment, and develop the patient's interdisciplinary individualized plan of care including short term and long term goals
- They will monitor and evaluate the member's progress towards meeting the goals and initiates appropriate interventions as necessary
- They will serve as an advocate for the member and act as a liaison between the member and other community based agencies, facilities, providers, and practitioners in coordination the member's care.
- They will assess and plan for medical necessity requests for outpatient services assuring that hospital discharge needs and non-acute services are appropriately provided/delivered to the members.
- Assumes a leadership role in monitoring appropriate utilization of the member's benefits and in coordinating with other payer sources.
- Understands, supports, and practices within the scope of the State of Pennsylvania Nurse Practice Act and the Case Management Practice Standards established by the Case Management Society of America
* Deals with chronically ill pediatrics
* Permanent position
* Day shifts: 8:30am - 5:00pm or 9:00am - 5:30pm
* May be able to work from home

REQUIREMENTS .
* Must have RN license in PA
* 3+ years of clinical experience and/or case management experience
* Pediatric, Utilization Review, and/or Managed Care experience preferred
* Will be eligible to obtain a CCM within 1 year of employment
