New Mexico announces a new statewide, coordinated long-term care program in which Evercare will be one of two health plans serving close to 40,000 Medicaid recipients. Evercare is a premier clinical care management organization, and collaborates with federal and state leaders to create new programs to serve this population. This program is the culmination of years of planning with the State of New Mexico, and we look forward to building partnerships here with our new members, providers and community organizations. Are you a nationally certified Nurse Practitioner interested in an autonomous, field-based role? As an NP in the Albuquerque, Santa Fe and surrounding areas, you would be assigned to a specific geographic area and provide care to our Medicaid population. This is an excellent opportunity to get in on the ground floor of a new program where you would get to work from home and in the field. Key responsibilities of the Nurse Practitioner include:Providing a holistic approach to care to our Medicaid members Collaborating with nurses and physicians to help keep members safe, healthy and independent Providing physical assessments to patients in their homes or assisted living facilities Assessing patients’ psycho-social, environmental and medication issues .
