In this role, you would enjoy an ideal blend of office and field work, providing direct care to hospice patients in their homes and skilled nursing facilities within the Montgomery and Baltimore County areas. Primary responsibilities include: * Providing on-site case management to hospice patients * Conducting admissions * Coordinating care with physicians, patients family members and staff at care facilities * Referring patients to outside resources as needed to ensure that all care needs are met * Documenting activities electronically on a company-provided tablet (company also provides a cell phone allowance and mileage reimbursement)
Qualifications The chosen candidate must possess: * Current, unrestricted RN license * Hospice experience * A clinical background involving med/surge and/or an adult population * Current drivers license, reliable transportation and insurance * MS Office skills * Associates or Bachelors degree (BSN preferred) We also prefer those with prior case management experience.
