We are searching for a skilled RN to oversee cases for hospice patients in their homes and skilled nursing facilities in the Vienna, VA area. The primary responsibilities of the Hospice Case Manager would include:
Visiting members in their homes or skilled nursing facilities within a 50-mile radius of our Vienna, VA office
Ensuring overall member care
Coordinating patient admissions
Working with patient primary care physicians, family members
Working with inter-disciplinary team to coordinate care
Qualifications
Required qualifications include:
A current, unrestricted RN license
A clinical background
Hospice experience
A current and valid driverīs license and auto insurance in VA
Computer proficiency
Preferred qualifications include:
Case management experience
A home care background
A BSN
