As a Case Manager, you would work closely with dedicated social workers and nurses in the New York City area to ensure the best care for our valued patients (primarily Chinese-speakers). Primary responsibilities include: • Managing cases for Medicaid-eligible AmeriChoice members with targeted chronic conditions• Assessing members' needs and coordinating home care, therapies, doctor visits, etc• Creating and implementing a care plan for each patient• Educating patients and following-up to monitor plan success and reduce unnecessary utilization• Visiting members in their homes with a team of caregivers as needed• Traveling to doctor's offices to collaborate with providers and members regarding the plan of care and case management efforts• Identifying and contacting members requiring outreach in order to complete care assessments and identify potential issues• Participating in rounds with the team and Medical Director • Attending monthly staff meetings in the office UnitedHealth Group offers a full range of comprehensive benefits, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan.
