We are searching for a motivated RN to join us in this unique field/home-based role. As a Case Manager, you would work closely with dedicated social workers and nurses in the New York City area to ensure the best care for our valued patients (primarily Chinese-speakers). Primary responsibilities include:
Managing cases for Medicaid-eligible AmeriChoice members with targeted chronic conditions
Assessing members' needs and coordinating home care, therapies, doctor visits, etc
Creating and implementing a care plan for each patient
Educating patients and following-up to monitor plan success and reduce unnecessary utilization
Visiting members in their homes with a team of caregivers as needed
Traveling to doctor's offices to collaborate with providers and members regarding the plan of care and case management efforts
Identifying and contacting members requiring outreach in order to complete care assessments and identify potential issues
Participating in rounds with the team and Medical Director
Attending monthly staff meetings in the office
The chosen candidate must possess:
A current, unrestricted RN license for the state of New York
Clinical experience
Bilingual Chinese fluency (Mandarin or Cantonese)
Case management experience
We also prefer:
Certified Case Managers
Experience working with a Medicaid population
Knowledge of regulatory requirements
