We are searching for a motivated RN to join us in this unique field/home-based role. As a Case Manager, you would work closely with dedicated social workers and nurses in the New York City area to ensure the best care for our valued patients (primarily Chinese-speakers). Primary responsibilities include: * Managing cases for Medicaid-eligible AmeriChoice members with targeted chronic conditions * Assessing members needs and coordinating home care, therapies, doctor visits, etc * Creating and implementing a care plan for each patient * Educating patients and following-up to monitor plan success and reduce unnecessary utilization * Visiting members in their homes with a team of caregivers as needed * Traveling to doctors offices to collaborate with providers and members regarding the plan of care and case management efforts * Identifying and contacting members requiring outreach in order to complete care assessments and identify potential issues * Participating in rounds with the team and Medical Director * Attending monthly staff meetings in the office
Qualifications The chosen candidate must possess: * A current, unrestricted RN license for the state of New York * Clinical experience * Bilingual Chinese fluency (Mandarin or Cantonese) * Case management experience We also prefer: * Certified Case Managers * Experience working with a Medicaid population * Knowledge of regulatory requirements UnitedHealth Group offers a full range of comprehensive benefits, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan.
