Nurse Case Manager

Responsibilities
Provide a collaborative, proactive approach to members with complex medical needs to meet their maximum potential with optimal clinical outcomes within coverage limits. Case managers serve as a point of contact for providers and assist members in navigating through the health care system and meeting their treatment goals.
Completes assessments and develops, implements, facilitates and monitors care plan
Documents activities and cost savings
Completes reports as required
Develops and maintains linkages with providers and internal staff
Requirements: .
WA state RN license and must obtain Idaho, Montana, Oregon and Alaska within 1 year of hire.
Minimum of 3-5 years clinical experience
Previous Case manager experience preferred
CCM preferred or within 2 years of hire
Experience in health insurance or managed care
