JOB MISSION SUMMARY

RESPONSIBILITIES:
-We are looking for a case manager to manage the west coast membership until 8pm EST so location is flexible
-There may be a rotation to 8pm occasionally for east coast applicants.
-Interact with all members of the healthcare team-both internal and external to Aetna including patients, families, physicians, providers.
-This position also has the potential to be a part of our work at home program if eligible per Aetna program requirements
-Serve as an integral member of the care management team
-Utilize clinical skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate inpatient healthcare management and effective discharge planning to improve the outcomes for members
-Obtain necessary information from providers and facilities concerning the care being provided to members
-Assess a member's clinical condition and ongoing medical services and treatments to determine medical appropriateness
-Guide the setting for medical intervention to achieve optimum length of stay for members; the early identification of continuing care needs to facilitate discharge to the appropriate setting and discharge planning & coordination
-Authorize and coordinate the required services, in accordance with the benefit plan
REQUIRED SKILLS:
-Registered Nurses with an active,unrestricted state licensure
-Case Management Certification preferred
-Consideration will be given to allow for obtaining the necessary experience and compentencies in the course of employment.
-Obtain and maintain CCM within 3 years of employment as a case manager
-3-5 years clinical experience in a hospital setting required and additional experience in an alternate setting such as home health or ambulatory care preferred
-Geriatric case management a plus in addition to working with the Medicare population
-Managed care case management and/or utilization management experience, 2 years preferred
-Strong customer service skills to coordinate service delivery including attention to customers ,sensitivity to issues, proactive identification and resolution of issues. Computer literacy including Excel and Word
-Ability to interpret and report data.
-Flex time and telework possible
-Some rotation to 8 PM EST possible.
