Here is a rewarding work from home position, ideal for a skilled RN capable of performing telephonic assessments and home visits to Medicare/Medicaid members. Our members will be in all different stages of healthcare and a self motivated RN is needed to work along side the patients' entire support team (physicians, family members, etc). Enjoy competitive benefits, growth potential and the chance to apply your talents to the following responsibilities: * Taking calls from members, vendors and home health providers * Obtaining resources and authorizations for members * Providing telephonic assessments to determine what services are needed after a hospitalization * Educating patients over the phone (regarding care) and making referrals for services * Using internal database to document status * Attending meetings on updated policies and tools * Handling home visits for an assigned caseload * Arranging member care (including treatment plans, monitoring their care, etc.) * Aiding members with the navigation of the Medicare and Medicaid complex care system of Medicare and Medicaid .
