Primary responsibilities include:
Conducting onsite clinical chart assessments to evaluate and identify delays in care
Facilitating discharge planning
Arranging alternative care services
Collaborating with physicians and other healthcare professionals
Determining care interventions if needed
Working with the Medical Director as needed
Negotiating to develop solutions for patients
Qualifications .
The chosen candidate must possess:
RN licensure in the state of NY
Strong clinical experience in an acute/inpatient setting
Discharge planning or case management experience
MS Office proficiency
Excellent communication skills
We also prefer:
Chart review experience
Managed care knowledge
Utilization review experience within an insurance company
Certified Case Managers
