Description Work from home while still making use of all of your nursing knowledge and experience!
Duties include:
Coordinating patient care services through phone interaction with patients, referral sources, visiting staff and patient care coordinators during the on-call period
Assess, plan, implement and evaluate nursing care provided to patients in the hospice/home care setting
Ensure that safe, quality are is delivered to all patients
Requirements This job requires a person who can work independently, while never forgetting that home health care is a team effort.
Other requirements include:
Nursing degree from an accredited school of nursing (Diploma or Associate Degree)
Current Alabama registered nurse license
Ability to make clinical evaluations and decisions based on measurable or verifiable criteria
Willingness to work in home environments of patients who may have values or standards different from your own
Ability to effectively communicate with patients, healthcare providers and others involved in the patient care
Respecting the patient s privacy and maintaining confidentiality
Current Alabama driver s license and automobile liability insurance
CPR Certification
