We are looking for two RNs to take on field-based roles in the Long Island region. As an Onsite Inpatient Care Advocate, you would enjoy flexibility, autonomy and no weekend or holiday hours. The hired individuals will concentrate on facilitating and expediting care for enrollees in acute hospital facilities and will work from home and in the field.
Primary responsibilities include: * Conducting onsite clinical chart assessments to evaluate and identify delays in care * Facilitating discharge planning * Arranging alternative care services * Anticipating patient progress and discharge planning * Discussing patient cases with physicians and other healthcare professionals * Visiting assigned facilities and receiving the daily census for those facilities * Participating in rounds with the Medical Director and discussing cases as needed
Qualifications
You must have a current, unrestricted RN license for the state of New York to be considered for this role. Clinical experience in an inpatient or acute setting is necessary. Candidates must also have experience in discharge planning, utilization review, case management or chart review. A background that involved working with utilization review for an insurance company or in a managed care environment would be ideal. Solid computer skills and the ability to build and maintain relationships is essential.
