Complex Nurse Case Manager - Peds

Our Care Coordination Dept. is currently seeking Complex Case Managers with a strong Pediatric background. Position responsibilities include identifying members who can benefit from KMHP s services and assess needs, develop plan of care, monitor and evaluate member s progress, serve as advocate for the member and liaison between member and community agencies, facilities, providers, etc. Qualified candidates must be a licensed PA RN, CCM preferred, 3+ years Case Management/clinical experience with a strong Pediatrics background. Experience in insurance (MCO), home care, assessments and care plan development a plus. Good computer, communication and organizational skills required. Potential to qualify for work from home after 6-9 months and certain competencies are met. Support and carries out the Mercy Mission.
Hours are 8:30a to 5p., four day work week (40 hr) and opportunity to work from home after 6 - 8 months.
