Our Case and Community Management Department seeks a skilled RN to coordinate care for elderly members in their homes and aid us in decreasing patient hospitalizations. This multi-faceted role would allow you to provide in home and telephonic clinical assessments regarding patient services, health conditions and overall patient care. Qualifications
Only RNs with a current, unrestricted license in their state of residence will be considered. Solid clinical experience (hospital, med/sure, home care, etc.) is required. A background in case management and care coordination of elderly members in various settings (home care, county, community health, etc.) is essential. Public Health Nurse (PHN) credentialing is preferred as is Medicare/Medicaid experience. A background managing benefits and working with low income members is highly preferred. Bilingual candidates (English/Spanish) are encouraged to apply. Experience with DME and working with vendors is a plus. A BSN is ideal. Computer skills are necessary.
