We are seeking a confident, self-motivated LVN to perform every aspect of case management for highly-acute members in the Austin area. This Care Manager position offers the opportunity to work from home and in the field, as well as the chance to make a tremendous difference in the lives of patients. Key responsibilities include:
Visiting Medicaid and Medicare members in their homes
Working closely with physicians, patients' families and other members of the care team
Developing treatments plans
Arranging and monitoring care
Assisting members in navigating the complex care system
Participating in telephonic conferences and attending team meetings
Documenting activities
You must have a current, unrestricted LVN license for the state of Texas to be considered. Clinical experience, preferably in med/surgery, ICU, home health, or involving Medicaid/Medicare, is required. A background in case management and/or long-term care would be ideal. Telephonic nursing and CBA experience would be helpful, as would knowledge of Medicaid/Medicare. Solid computer skills and excellent self-management abilities are essential. Candidates who speak Spanish or Vietnamese, in addition to English, are preferred.
