In this role you would work from home and in the field, meeting with staff in acute hospital facilities to coordinate and expedite care. Conducting onsite clinical chart assessments
Identifying and evaluating delays in care
Facilitating discharge planning
Arranging alternative care services
Discussing cases with treating physicians and other healthcare professionals
Determining need for intervention, and discussing with Case Managers and physicians
Participating in rounds with the Medical Director
Requirements
You must be a currently licensed RN, PA or APRN in the state of New Jersey to be considered for this role. Recent clinical experience in an inpatient/acute setting is required. Discharge planning and/or chart review experience is preferred. A background that involved working with utilization review for an insurance company or in a managed care environment would be ideal. Solid computer skills and the ability to build and maintain relationships will be essential. M&R familiarity is desired.
