Description
EXCITING OPPORTUNITY for PT and FT RNs in the Chicago area.
PT &FT RNs needed for home-based case management.
Help reduce hospitalizations, promote wellness, and reduce overall cost of healthcare for enrolled patients. Team approach to community-based case management with emphasis on home care services putting nurses in lead role. Will be instrumental in communicating with physicians and managing high risk, chronically ill patients. Key in screening, educating, managing and coordinating care, promoting self-management and maintaining relationships with patients and interdisciplinary team members. Components of the model include comprehensive assessment, development of individualized care plan, and use of tested care management strategies. IL RN License Required. Exp with adults, geriatrics and chronically ill populations a must. Internet access and computer skills a required. Immediate openings. Bilingual English/Spanish a plus.
Minimum Requirements:
1. IL RN License Required
2. Successful credentialing as a registered nurse in accordance with state laws.
3. Experience with chronically ill geriatric population a must.
4. Familiarity with home care, developing individualized care plans, and motivational teaching experience.
5. Bilingual English/Spanish a plus
6. Computer skills (email, Microsoft word) and high speed internet access a must.
