Job Description:
The position of Provider Services Manager will report to the PSM Team Leader and will be responsible for providing assistance to physicians affiliated with our health plan customer, Cariten, with regards to their patients with chronic disease. As a representative of the corporate clinical department, the PSM will travel locally and regionally to meet with these physicians.
Responsibilities include:
* Market Healthways' care enhancement program, securing initial and ongoing buy-in/support, and eliciting feedback for use in program improvement.
* Assess/develop plan to support the physicians' needs for improving the clinical and financial outcomes for their patients with a diabetes and/or heart failure, utilizing available tools and developing new tools/processes as needed.
* Support physicians and staff in the delivery of Standards of Care related to these specific chronic diseases.
* Provide for and review with the physician the results of individual panel outcomes, including both financial and clinical outcomes.
* Coordinate educational offerings for physicians, nurses and staff.
* Review data provided by the Care Coordinator, identifying trends/opportunities for intervention and improvement, developing a plan to address the opportunities with the appropriate persons.
Experience and Qualifications:
The qualified candidate must be an RN with a current Washington State license (Bachelors degree required) and have 3-5 years recent experience in inpatient or outpatient cardiac and/or diabetes disease management. Desire a self-starter with demonstrated physician relations success, analytical and problem solving skills, and presentation skills. Experience in a managed care organization strongly preferred; physician relations or physician sales experience desired. Computer database management and data analysis skills are necessary. Work from home-based office, with 80% of the workweek traveling to physician sites, utilizing own car. Car allowance and mileage reimbursement provided. Occasional air travel will be required.
