Excellent opportunity for a hands-on Home Care Nurse to provide homecare to patients located in the Brooklyn and Queens specialized facilities. Nurses will receive a laptop and cell phone. Will be able to work from home with occasional visits to the main office. .

Specialized health care facility .
Responsibilities of the Home Care Nurse include:
Providing homecare to hospice patients
Coordinating and providing high quality multidisciplinary health services for patients
Working under moderate supervision
2+ years of homecare experience
RN
AAS .
Hospice experience
Access to a car and valid driver's license
Bilingual-Spanish .
