The Medically Complex Children s Program of MENTOR Maryland at 5750 Executive Drive, Catonsville, MD, is seeking to fill a Case Manager (LSWA / LGSW) opening. The Medically Complex program provides medically trained foster homes and case management for children with chronic health conditions, developmental delays/mental retardation, and/or physical disabilities, ages 0-21. The majority of the 80+ children we serve are under the age of 3. Each two-parent foster home has a primary stay-at-home caregiver. This program strives for permanency where reunification is not possible: we have finalized over 40 adoptions by our foster parents since 2002.
The Case Manager for the Medically Complex Children s Program will:
1. Provide case management to a caseload of 7 children placed throughout central Maryland (up to 1 hour travel time from our Catonsville office)
2. Conduct weekly home visits to foster homes, monitor quality of care to child, provide support to foster parent
3. Conduct monthly visit to child s educational site
4. Collaborate with health care providers at specialty hospitals and in the community
5. Write Individual Habilitation Plan and updates quarterly
6. Attend weekly Treatment Team Meetings with all program staff
7. Forge professional relationship with Custodial Agency worker (DSS) and child s attorney
8. Assist with providing pre-service and in-service educational sessions for foster parents
9. Collaborate with other MENTOR staff: Intake Coordinator, Foster Parent Recruiters, and Nurse Specialists
10. Receive weekly on-site supervision by LCSW-C

Requirements
Current license at the BSW level (LSWA) or MSW level (LGSW) is mandatory. Experience in case management, pediatric health care, or foster care services is preferred. Also preferred is a proficient knowledge of MS Word, Excel, and other common computer programs.
