Description
Pennsylvania's largest healthcare insurance provider is looking for experienced Pediatric and Geriatric Registered Nurse. Full positions with the company within an office environment. Hours are 8:30am-5pm, Monday thru Friday.
After 6-8 months there is the opportunity to qualify for work from home if certain competencies are met. Great opportunity for a RN looking for a change from the hospital or typical case management position.
Job Duties include:
*Identifying members who will benefit from services
*Perform a comprehensive member assessments
*Develop individualized plans for members that includes short and long term goals
*Monitors and evaluates meeting set goals and makes changes as necessary
*Serves as an advocate for the member and coordinates care with providers, facilities, etc.
*Assess requests for outpatient services
*Ensure hospital discharge needs and non-acute services are appropriately provided/delivered to the members
*Assumes a leadership role in monitoring appropriate utilization of the member s benefits.
*Understands, supports, and practices within the scope of the State of Pennsylvania Nurse Practice Act and the Case Management Practice Standards established by the Case Management Society of America (CMSA).
*Support and carries out the company mission
Requirements
*Valid, current PA RN license
*1+ year experience in Case Management
*At least 3 years clinical experience specifically in either Pediatrics or Geriatrics
*CCM certification preferred
*Experience with utilization review, discharge planning, or home care highly preferred
*PC capabilities
*Organizational skills
