Job Description .
Medical Case Manager:
Work from Home
**Must be Bilingual Spanish**
Coordinates resources and creates flexible, cost-effective options for catastrophically or chronically ill or injured individuals on a case-by-case basis to facilitate quality individualized treatment goals, including timely return to work if appropriate. Provides medical case management to individuals through coordination with the patient, the physician, other health care providers, the employer, and the referral source and coordinates Independent Medical Evaluations. Provides assessment, planning, implementation, and evaluation of patients progress. Evaluates patients treatment plan for appropriateness, medical necessity, and cost effectiveness. Implements care such as negotiating and coordinating the delivery of durable medical equipment and nursing services. Devises cost-effective strategies for medical care.
Attends doctors and attorneys visits. Attends hospital and/or long-term facility discharge planning conferences. Assesses rehabilitation facilities for appropriateness. Performs or coordinates architectural assessment of patients home. Required to read extensively. Required to prepare organized reports within a specified time frame. Required to use telephone extensively.
Required to research medical and community resources for patients with catastrophic or chronic diagnoses, such as but not limited to, AIDS, Cancer, spinal cord injury, diabetes, head injury, back injury, hand injury, burns, et cetera. Heavy travel required. Minimum Billable Standards are 1.8 to 2.2 hours per case, per week, working with an average of 25 cases per month. Requires regular and consistent attendance as well as thorough, organized and consistent reporting per special handling guidelines.
Comply with all safety rules and regulations during work hours in conjunction with the Injury and Illness Prevention Program (IIPP). Additional duties as required
Essential Job Duties: Comprehensive Case Mgmt.
Education/Training Requirements: Applicable state licensure and professional case management certifications.
