Care Manager Job Description:
Aerotek Scientific is seeking an RN/Care Manager for a Medical planning company.
The person will be dealing with the geriatric population 99% of the time. Experience working with this group in the therapeutic area is a non-negotiable (CHF, COPD, Diabetes, ESRD). Person will be involved with a start up group in this area. They are looking to open up a clinical facility much like a doctors office soon. Person will be able to work from the office, home and on the road at least half the time. They have seen growth from 100 to 400 employees in the last 2 years ? a lot of promotion potential. They are looking for an ambitious, go getter type that does well in a fast paced environment.
The Care Manager is primarily responsible for performing the day-to-day disease management interventions for chronically ill patients in Special Needs Plan.
Required Skills for Care Manager Job:
RN LICENSE
3-5 YEARS OF CLINICALEXPERIENCE WITH KNOWLEDGE OF MANAGED CARE
CASE MANAGEMENT
CERTIFICATION OR ELIGIBILITY PREFERED
DISEASE KNOWLEDGE OF CHF
ESRD
DIABETES AND COPD
