We are looking for several self-directed Nurse Practitioners to provide primary care services to elderly residents in long-term care facilities. Primary responsibilities include:
Managing chronic and stable conditions of patients
Working collaboratively with physicians to develop, discuss, and monitor care plan
Participating in voluntary on-call rotation
Interacting regularly with the care staff at the facilities
Documenting all activity
Attending rounds with physicians
Participating in care conferences with patients and their families
Attending monthly meetings
Qualifications .
This role requires:
National NP certification in Geriatric, Adult, or Family care (Geriatric certification highly preferred)
Excellent communication skills
Computer proficiency
Highly valued assets include:
Acute care and direct care experience in a long-term care setting
Familiarity with Medicare regulations
Recent NP graduates with RN experience in a long-term care setting, skilled nursing facility, and/or with a geriatric population will also be considered.
