THE BEST WAY TO PREDICT YOUR FUTURE IS TO CREATE IT...... Our Client is a hospital in Austin, TX and is seeking a Registered Nurse to serve as their Transplant Coordinator. POSITION SUMMARY: Working cooperatively as a member of the transplant team, the transplant coordinator is responsible for coordinating all the activities related to pre- and post-transplant patients. These activities include evaluating potential recipients and living donors; educating potential recipients, living donors, and post-transplant patients; providing consultative services and education to nursing services during the transplant recipient’s hospitalization; and coordinating follow-up care of the transplant patient. ESSENTIAL JOB RESONSIBILITIES: Note: Job responsibilities are divided into pre- and post-transplant and coordinators are hired to focus on one primary role; however, coordinators are cross-trained in both areas of transplant care for coverage purposes. Pre-Transplant: 1. Coordinates assessment and evaluation of potential transplant recipient and living donor. · Prioritizes data collection according to the patient’s condition or needs. · Obtains physician approval for the patient as a transplant candidate. · Provides data coordinator with all information necessary to meet UNOS and Medicare regulations. · Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs. · Assists in development of Long Term Care Plans for each patient. · Coordinates all surgical arrangements for living related donor transplants. 2. Provides education about transplantation to patients and their families. This education include: · Evaluation process/UNOS listing process · Risks and complications of transplantation · Hospital admission/surgical procedure · Medication: effects, side effects, dosing, interaction with food · Maintenance of diary · Self-care AT HOME: vital signs, intake and output, weight · Follow-up appointments · Prevention of infection 3. Assess potential learning problems (language barrier, readiness to learn, etc.) and structures teaching sessions to meet individual needs. 4. Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs. Post-Transplant: 1. Assists nursing units in the care of all transplant patients to ensure proper admission and care of the hospitalized patient. · Reviews charts, test results and medications. · Makes rounds on hospitalized patients. · Communicates issues to physicians. · Provides discharge teaching to patients and family members. · Assists in arranging discharge medications. 2. Coordinates and assures thorough and consistent follow-up care of the transplant recipient with the nephrologists, transplant surgeons, and other care providers. · Compiles and reviews data prior to clinic visits. · Follows the transplant recipient in the transplant clinic. · Collects and evaluates outstanding data after each visit. · Provides data coordinator with all information necessary to met UNOS and Medicare regulations. General Responsibilities: 1. Educates staff and community in the area of transplantation. 2. Rotates 24-hour call duty for donor screening, coordination of transplant team for cadaveric transplants, and triage of patient calls. 3. Skills/Requirements EDUCATION AND EXPERIENCE: Required: Nursing degree FROM an accredited institution required; 1-2 years clinical nursing experience. Preferred: 1 year critical care nursing experience; experience with renal transplantation or dialysis, case management, critical care, renal urology preferred. BSN LICENSES AND CERTIFICATES: Required: Current licensure in Texas as a Registered Nurse Certification in basic cardiopulmonary resuscitation Preferred: Certified Clinical Transplant Coordinator preferred SALARY: Above salary scale is a national projection so it will vary depending on location. Specific information will be provided upon preliminary screening with our client.
