Job Description
Exciting opportunity to play a key role with an innovative and growing company providing juvenile justice continuing care program. We are a mission-driven organization with a history of successful programs throughout Pennsylvania. We are seeking an experienced Case Manager to join our Reintegration Program in Philadelphia. Outcome studies have demonstrated the success of this program at redirecting troubled youth and assisting families.
Case Managers are tasked with assisting delinquent youth in benefiting from residential placements and in making a successful return to family and community. Our program does not operate residential programs but works with youth while they are in placement to focus services on the reality of home and community and to do earlier, and more concrete, planning for their reintegration to home and community. While youth are in placement, our Case Managers see families to help resolve problems and develop their strengths to support their child's successful reintegration. Upon return home, we provide suport and supervision for 3 months to help youth and families achieve successful outcomes.
Services are provided in home and community and our staff work flexible non-conventional hours to do "whatever it takes" to achieve positive outcomes.
Must have a Bachelor's Degree in related field and at least 2 year's experience working with juvenile delinquents, preferably in a blend of residential and community settings.
Successful candidate will be a diligent and proactive casemanager balancing an ability to motivate and inspire youth to positive behavior with timely and high quality reporting. Should be familiar with community resources in Philadelphia. Good communication skills and an abililty to work with diverse population and maintain good relationships with Juvenile Probation, residential providers and multiple stakeholders.
Some travel required to in-state placement agencies.
Spanish fluency and experience in family counseling very desirable.
