We have a great opportunity, reporting out of our Norcross office, to become our newest Manager of Case Management. This will be a primarily field-based position where you will have the opportunity to work from home when not in the field (you will only be required to come into the office one day per week for meetings). Primary responsibilities include: Overseeing the day to day actives of Case Managers Conducting audits and reviews of nurses documentations and interventions preformed by the nurses Identifying, training and orientating new and existing care management staff Maintaining staffing ratios within established benchmarks and meeting program goals Providing coaching to Case Managers for achievement of team goals and running team meetings Managing any compliance issues and acting as a resource for the team Working one on one with the team and meeting with physicians and patients
Qualifications Requirements for this role are: A current and valid RN license in Georgia Clinical experience (med/surg, ICU, chronic disease management) Management or supervisory experience Basic skill with Microsoft Office products
Preferred assets include: Preceptor experience Customer service experience UR experience Case management background Direct DON experience - 1 year Staff development, training or orientation experience Home care Management experience Medicare Benefits knowledge or experience working with Medicare benefits A Master's degree
