Are you an RN looking for a break from direct patient care? We may have the perfect opportunity for you to become our newest Clinical Care Manager working in the Philadelphia (and surrounding counties) territory. This position will entail local travel about 50% of the time while the other half of your time will be spent working from your home. Primary responsibilities include:
Improving access to health and well being services
Promoting the quality of care for the patient
Working with a wide variety of patient illnesses to manage and coordinate care
Traveling, as needed to patients homes and physicians offices
Conducting ongoing case management and preparing monthly status reports
Traveling locally within a 50-mile radius
