We're seeking a new Case Manager to provide telephonic disease management services for a variety of members in one of our employer groups. This is a highly autonomous role that offers a great schedule (no weekends or holidays), a supportive team environment, and the competitive benefits you would expect from a Fortune 50 organization. Primary responsibilities include:
Making outbound calls to members and facilities and taking inbound calls to provide case, risk and disease management
Utilizing Milliman criteria to determine if patients are in the correct hospital setting
Working with onsite Case Managers, physicians and other care staff to ensure that discharge plans are in place
Making ""welcome home"" calls to ensure that discharged members receive the necessary services and resources
Educating members on their discharge care plan instructions, medication management, etc.
Coordinating services as needed (home health, DME, etc.)
Developing care plans and assisting consumers with obtaining access to care, providers and lifestyle changes
Making referrals to partner programs
Collaborating with the Team Lead and Medical Director on complex cases
Experience:
Required qualifications include:
Current, unrestricted RN license for CA
Recent acute care, hospital or direct care experience, OR experience as a telephonic Case Manager for another insurance company
Ability to type while speaking on the phone
Solid computer skills
Preferred qualifications include:
Home health case management experience
Experience in utilization review, concurrent review and/or risk management
Exposure to discharge planning
BSN
