Superb opportunity for a registered nurse (RN) with experience working with diabetes to become a Case Manager for a respected managed care organization. The Case Manager assists with the development, organization, implementation, monitoring and evaluation of activities related to the outcome management of assigned patients. The qualified candidate will act as a resource, consultant and educator in the planning and delivery of patient care within the Network. .
Not-for-profit managed care organization .
The Case Manager will:
Manage care of patients with HIV or diabetes
Perform preadmissions, concurrent and retrospective reviews to evaluate appropriateness of admission, length of stay, utilization of resources and usage of other service post encounters
Document all interventions and all telephone encounters with providers, members, and vendors in the appropriate system in accordance with established documentation standards
Document and close all authorizations and make a determination as to whether the requested services should be paid or referred for possible denial to insure expeditious claims processes
2-10 years of experience as a RN
Experience with HIV or diabetes CM
Strong Microsoft Office skills
Experience with Medicaid care and treatment
Case management/utilization management
Managed care experience .
