DESCRIPTION .
This position is clinical. The Tele Health Nurse practices nursing care within the standards of the state Nurse Practice Act using the nursing process. The Tele Health Nurse provides care to individuals or defined patient populations through the use of telecommunications in accordance with computer-based algorithms, protocols, or guidelines. The Tele Health Nurse uses critical thinking and communication skills to assess, plan, implement, educate, and evaluate patient outcomes. The Tele Health Nurse will facilitate care between the patient, family, and other health care resources.

Key Responsibilities (List five to seven essential responsibilities in priority order):

- Demonstrates sound clinical judgment in accordance with the Nurse Practice Acts.
- Adheres to nursing standards and company policies and shared principles.
- Accountable for professional practice, behavior, and licensure initiatives.
- Uses the nursing process to identify patient care needs, risk and safety issues, educational opportunities, and appropriate health care resource referrals.
- Processes calls based on Quality Management guidelines.
- Demonstrates compliance with all product-specific performance metrics.
- Demonstrates compliance with all policies and procedures.
- Initiates the appropriate identification and communication of issues, complaints, compliments, and risk concerns.
- Demonstrates excellent customer service and organizational skills. .
REQUIREMENTS .
Education/Training
- Associate degree or diploma in Nursing. Bachelor s degree in Nursing preferred.

Business Experience
- Experience in a clinical call center preferred.

Specialized Knowledge/Skills
- Current RN license in good standing.
- 3-5 years recent acute care nursing experience.
- Excellent communication and customer relation skills.
- Excellent clinical documentation and writing skills.
- Excellent critical thinking, decision-making, and problem-solving skills.
- Excellent computer/keyboard/mouse skills.
- Excellent multi-tasking skills with demonstrated ability to simultaneously navigate a computer system and phone system while effectively communicating with patients.

Working Conditions:
Environment (Office, warehouse, etc.) Office environment.

Physical Requirements (Lifting, standing, etc.) Ability to use a computer keyboard, mouse, telephone, and telephone headset. .
