Exciting opportunity for an experienced case manager to work independently as an on site case manager
Responsibilities:
Perform on site case management for assigned cases
Interact with entire interdisciplinary team, including: social workers, in house case managers, physicians and administrative staff
Work independently and autonomously from a home office - flexible position!!!!
Specialty positions available, including HIV and Diabetes
SALARY: to $70,000 plus great perks and benefits!
