Specialized healthcare facility in need of experienced and flexible Home Care Nurses who are committed to providing high quality multidisciplinary health services care to patients. Candidates must be organized team players willing to travel throughout the boroughs of New York.

Community-based specialized healthcare facility .
Home Care Nurse must be able to:
Provide home care to hospice patients
Coordinate and provide high quality multidisciplinary health services for patients in the program consistent with agency philosophy, policy, goals and objectives and standards of nursing practice
2 years of RN experience
Home care experience
Applied Science degree in nursing .
Hospice experience
Bilingual-Spanish
Have access to a car and valid driver's license .
