We are searching for a friendly, caring nurse to become our newest Care Coordination RN on our Long Beach, CA team. Primary responsibilities include:
Managing outpatient cases and making telephone contact with patients
Providing education to patients upon discharge from the hospital on diet, medication and benefits issues
Conducting health assessments and assisting patients in making decisions on the next steps in their care
Coordinating DME and at-home nursing for recently discharged patients
Participating in weekly case meetings to discuss unique situations or to escalate cases
Requirements
The chosen candidate will possess a current and unrestricted RN license in California. Candidates must have recent direct care experience in an acute care or hospital environment or telephonic case management experience for a large insurance company. Home health case management experience is preferred. Utilization review, concurrent review and/or risk management experience is a big plus.
