Give your job title, resignation, and effective date, if possible. If you prefer, this one statement is all you need.
Effective May 1, I will assume the position of marketing manager at Universal Builders in Omaha. Therefore, I’m offering my resignation here, effective April 30.
Please accept my resignation as systems programmer, effective May 13.
I’ve recently decided to leave my position as auditor with Utco. My last day here will be April 30, although I do have some flexibility in that effective date should the situation warrant my staying on a few weeks longer.

State your reasons, if you can do so sincerely and truthfully without repercussions. If you prefer, omit any statement of reason altogether or give a vague, general comment.
I will be assuming a position more in line with my graduate studies and current interests.
